When you use the Request a Ride or Contact forms, we collect the details you provide, such as your name, phone number, email address, pickup and destination locations, requested dates, and a brief description of the transportation assistance needed.

Please do not include diagnoses, patient names, or other detailed medical information in these forms. If we need additional information to arrange a ride, we will discuss it with you directly by phone.

How we use it

We use the information you share to review your request, follow up with you about availability and next steps, and, if a ride is confirmed, to provide the transportation you asked for. We do not sell your information.

Who can see it

Your information is available to LifeLink team members who need it to respond to your request. If you are contacting us on behalf of a healthcare organization, we may use your details to discuss a referral process with you.

Your choices

You can ask us to correct or delete the information you have shared by contacting us by email or phone using the details at the top of this website.
